The form may be Faxed, emailed or mailed, but no phone messages or requests without a form will be accepted.
Please take the time to fill one out, as this is how we track, record and report our usage and it will affect what we are given to use in future seasons.
If you do not see a spot you wish to practice, please contact Mike at Mike@StVrainFC as email is the easiest way to track a particular conversation during this busy time of year.
